Hello, Voodoo Performers.

Getting paid will be easy, just fill out the forms below and post-performance you’ll receive a direct deposit to your banking account just as soon as the funds have cleared on my end.

Be sure to fill out the W9 as well, so we can complete the payments and track you down at the end of the year (come tax time).
